Release of Liability
Submission of this form stipulates the following:
I agree to hold the organizers of this event and their agents, heirs, and assigns harmless from all suits, claims or demands of every kind and character arising out of and/or in connection with this class, workshop, lesson or event.
I hereby authorize the reproduction, sale, copyright, exhibition, broadcast and/or distribution of any event still image or video photography without limitation and further agree not to reproduce, distribute or sell videos or still photographs of any class, workshop, lesson or event.
I understand the physical risks of participating in special events, dance classes, dance workshops, dance lessons, self-defense workshops, acting classes, social dances, practice sessions or any other Follow My Lead activity and assume full responsibility for any injury or personal damages resulting from my participation in any Follow My Lead activity.
I certify that I am 18 years of age or older. Stipulations apply to Legal Guardian if applicant or attendee is under 18 years of age.
I take responsibility for my own risk.
Follow My Lead Dance Studio is not responsible for exposure to Covid or any other illness:
You are entering and participating at your own risk. Social dancing may put you in close proximity to other people. Being around other people will always carry risk. If this worries you or makes you uncomfortable, please come back at a later date.
Follow My Lead Dance Studios and/or the instructors that teach here are not responsible for any injury or illness that may happen due to your participation. You are dancing at your own risk.
If you are sick, please come back when you are feeling better.
Masks are not mandatory. If you are wearing a mask, please use caution and listen to your body. Exercising with a mask may lead to lightheadedness, headaches or fainting.
NOTE: Lifts or anything else deemed “Crazy Stuff” are not allowed without express written permission from Crystal Reiber.
